Please arrive 10-15 minutes early prior to an initial appointment. This time will be used to fill out the necessary clinic intake forms.

Walk-ins:
Due to COVID-19 regulations, we ask that walk-in patients please call or email ahead.

In-Home Visits:
Home-visits are available in certain circumstances upon request. Please call the clinic for more information.

Cancellation Policy

If an appointment is not able to be kept, we insist that you notify us as soon as possible. Our cancellation policy requires 24 hours notice prior to the time of the scheduled appointment. If cancellation is not met within the 24 hours, a fee of 50% of the appointment cost will be charged to the individual's account to be paid before the next appointment is booked.
